What is a Family Health Team? (FHT)

A Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you – the patient.

Are there any fees associated with the participation in programs and services offered?

All programs and services are covered by Ontario Health Insurance Plan. (OHIP).


Do I require my health card with each visit?

You will need to present your health card at the reception counter with each visit.  This will ensure that your health card information is up-to-date in our system.  Please advise us if there has been a change in address and/or telephone number as well.


Are there After Hours Clinics?

Patients of our clinic have access to same day urgent care appointments during the day and access to an on call physician weekday evenings. Patients also have clinic walk-in access on Saturday and Sunday .  Please visit our contact page for details.

If I am seen by an Inter-Professional Team Member of the Family Health Team, will my family physician be aware of my visit and receive any relevant results?

Yes.  Our Family Health Team uses an EMR system (Electronic Medical Records), and as a result, all information in our system can be viewed by your physician.


Where is the closest lab to complete blood work?

The closest lab is Life Labs which is conveniently located in our building.


What are Nurse Practitioners and what are their qualifications?

Primary Health Care Nurse Practitioners (PHC NPs) are extended class registered nurses.  They are experienced nurses with additional nursing education which enables them to provide individuals, families, groups and communities with health services in health promotion, disease and injury prevention, cure, rehabilitation and support.  The NP is an advanced practice nurse, functioning within the full scope of nursing practice.
